Next Minimum Day - Early Release Wednesday, May 16th
Kindergarten Registration
Parent/Legal Guardian must be present and bring with them:
● Proof of residency (utility bill, etc.)
● Child’s Social Security Card (not compulsory)
● Birth certificate, baptismal record, or statement of attending physician must
be presented to verify age of child (child must be five years old on or before
November 1, 2012 to register for Kindergarten).
● Completed physical exam dated after March 1, 2011.
AN APPOINTMENT FOR AN EXAM WILL NOT BE ACCEPTED.
● California State Law requires the following immunizations and dental assessment:
Polio –4 doses (3 if last dose on or after 4th birthday)
DPT - 5 doses (4 if last dose on or after 4th birthday)
MMR –2 doses (first dose after 1st birthday)
Hepatitis B –3 doses
Varicella –1 dose (or proof of disease documented by health care provider)
Tuberculin Skin test given, read and documented on or after March 1, 2011
Or - Provide signed physician release statement from medical provider
Dental Assessment: Dated after August 15, 2011 - Forms available in school office
